Summary:

This position is responsible for oversight, development and implementation of the organization’s foundation and government grants portfolio. The primary purpose of this position is to advance service and programmatic goals of Heartland Health Centers and support the financial sustainability of the organization.

Essential Duties and Responsibilities:

  • Writes and submits government and foundation grant proposals. This includes creating proposal narratives, data measures, budgets and other necessary documentation for the applications. 
  • Coordinates with relevant staff to ensure program compliance with requirements of awarded grants, including site visits, allocation of expenses, data collection and submission of required funder reports. 
  • Facilitates the acquisition of new government foundation grants from grant research and identification, communication with prospective funders and coordinating site visits. 
  • Develop and maintain a grant prospect database. 
  • Processes grant awards, writes and sends thank you letters for awarded grants, and maintains accurate and up-to-date records of the status of each grant, deadlines for renewals and reports, and changing priorities of funding agencies. 
  • Acts as liaison between Heartland Health Centers and funders to maintain and steward the relationship. 
  • Staffs two internal committees: a grants committee and grants finance committee. 
  • Participates in monthly leadership team meetings of the organization and other duties as assigned.

Education and/or Experience:

  • Bachelor's degree, and at least three (3) years of experience in grant writing, program planning and development. 
  • Experience working in a health care environment is a plus.
